Common signs and symptoms of urinary tract infections
- Lower UTI: Dysuria, urgency, frequency, nocturia, suprapubic heaviness, gross hematuria
- Upper UTI**: Flank pain, fever, nausea, vomiting, malaise
- Notice pyuria is not listed
- *More systemic symptoms
What happens when you treat asymptomatic bacturia?
People are more likely to become symptomatic! Don’t treat!
Who should be screened for asymptomatic bacturia?
Pregnant women (reduces incidence of pyelonephritis and preterm delivery)
People undergoing urologic procedures (reduces incidence of septic complications)
*Screen and then treat them if positive.
How long should you treat complicated UTI and pyelonephritis?
7-14 days, unless levaquin 750 daily, then 5 days.
If a drug has a Cmax/MIC PK/PD profile (like AG’s), then what is a good target Cmax/MIC?
100 x MIC
How long to treat uncomplicated community acquired UTI?
Men: 14 days
Women: 7 days
When should you use FQ’s in UTI?
When you don’t have any good alternatives. Causes Tendon problems, muscle/joint, nervous system, blood sugar, mental health, cardiovascular (aneurism), issues.
What MDRO’s need to be reported to the NHSN?
MRSA, VRE, MDR acinetobacter, CRE
What MDRO’s need to be reported to CMS?
MRSA, C. diff.
