Uncomplicated cystitis in women

PO Nitrofurantoin 50 mg QDS x 3-5 days
PO Augmentin 1 g BD or 625 mg BD x 5-7 days
PO Fosfomycin 3 g single dose

Complicated cystitis in women

Answer 2

Treat as per uncomplicated cystitis with longer duration (7-14 days)

Question 3

Community-acquired pyelonephritis in women, non-severe

Answer 3

PO Augmentin 1 g BD or 625 mg TDS x 10-14 days
PO Cefuroxime 250-500 mg BD x 7-10 days

Question 4

Community-acquired pyelonephritis in women, severe

Answer 4

IV Augmentin 1.2 g q8 hours or
IV Cefazolin 1 g q8 hours

Consider adding IV/IM Gentamicin 5 mg/kg

Treat for 10-14 days

Question 5

Nosocomial pyelonephritis

Answer 5

IV Cefepime 2 g q12 hours + IV Amikacin 15 mg/kg/day or
IV Imipenem 500 mg q6 hours or IV Meropenem 1 g q8 hours or
PO Ciprofloxacin 500 mg BD or
PO Levofloxacin 750 mg OD

Treat for 7-14 days

PO Ciprofloxacin 500 mg BD x 10-14 days or
PO Co-trimoxazole 960 mg BD x 10-14 days

Question 8

Catheter-associated UTI, severe

Answer 8

IV Cefepime 2 g q12 hours + IV Amikacin 15 mg/kg single dose or
IV Imipenem 500 mg q6 hours or IV Meropenem 1 g q8 hours

Treat for 7 days if adequate response in 72 hours, if not 10-14 days

Question 9

Catheter-associated UTI, mild

Answer 9

PO Levofloxacin 750 mg OD x 5 days

Question 10

Catheter-associated UTI, in female patient < 65 y.o., nil upper UTI symptoms and catheter removed

Answer 10

PO Co-trimoxazole 960 mg BD x 3 days
