Urinary Antiseptics Flashcards
15.29
Nitrofurantoin - Class
Urinary antiseptic
Nitrofurantoin - MOA
Unchanged drug (~40%) is excreted into urine; bacteria in urinary tract reduce drug to highly reactive intermediates that damages DNA; More active at low pH
Nitrofurantoin - Active against…
Effective against gram-positive enterococci and gram-negative E. colis
Bacteria resistant to Nitrofurantoin (4)
Urease forming bacteria Proteus
Pseudomonas, Enterobacter and Klebsiella
Methenamine - Class
Urinary antiseptic
Methenamine - MOA
Metabolized to toxic formaldehyde spontaneously in low urine pH, but not in plasma at pH 7.4
Methenamine - Tx
Prophylaxis of chronic UTI
What bacteria should not be tx w/Methenamine?
Do not use w/ urease forming Proteus –> alkalinizes pH and prevents drug activation
