How do you classify arthritis?

A

Arthritis is a broad term for any joint disease which involves inflammation
Can be categorised:

  1. Degenerative
    - osteoarthritis
  2. Inflammatory
    - rheumatoid arthritis
    - psoriatic arthritis
    - gout
    - pseudo gout
    - SLE
  3. Infective
    - septic arthritis

What is osteoarthritis?

A

Osteoarthritis is a degenerative condition of ARTICULAR CARTILAGE

It is characterised by destruction of articular hyaline cartilage
This leads to:
- remodelling of periarticular bone as the joint tries to repair itself
- structural alteration of the joint incl osteophytes, sclerosis and subchondral cysts
- decompensation and joint failure as natural repair mechanisms fail to cope
- inflammation and synovitis with symptomatic joint

What is rheumatoid arthritis?

A

Rheumatoid arthritis is an autoimmune inflammatory condition affecting SYNOVIUM

It is characterised by triad of

  • persistent synovitis,
  • systemic inflammation
  • presence of autoantibodies (RF / ACPA)

Autoimmune reaction occurs in response to antigen
This triggers formation of immune complexes and activation of autoantibodies which leads to an inflammatory response within the synovium
- inflammatory cascade leads to up regulation of TNF alpha /IL-1
- synovium becomes thickendped and inflamed - PANNUS
- synoviocytes invade cartilage and tendons
- cause joint destruction and weakening/attenuation of tendons which can lead to rupture.

A

Darrach’s procedure is an excision arthroplasty of the distal ulna
Steps are:
Longitudinal dorsal incision over distal ulna
Elevate skin flaps protecting dorsal branch of ulna nerve
Raise extensor retinaculum as flap
Open DRUJ capsule to expose ulna head
Limited subperiosteal resection of distal ulna (
