Action: Extend, Adduct, and Internally Rotate Shoulder
Innervation: Thoracodorsal n.
Teres Major
Origin: Inferior angle of scapula
Insertion: Intertubercular sulcus
Action: Adduct, Extend, & Internally Rotate Shoulder
Innervation: Lower subscapular n.
Levator Scapulae
Origin: Transverse processes of C1-C4
Insertion: Superior aspect of medial scapular border
Action: Elevate scapula & Inferiorly tilt glenoid
Innervation: Dorsal scapular n.
Rhomboids
Origin: Spinous processes C7-T5
Insertion: Medial Scapular border below spine
Action: Retract scapula & Rotate glenoid inferiorly
Innervation: Dorsal Scapular n.
Serratus Anterior
Origin: Lateral most aspect of ribs 1-9
Insertion: Ventral surface/Medial border of scapula
Action: Protract scapula & Superiorly tilt glenoid
Innervation: Long thoracic n.
Deltoid
Origin: Scapular spine, Acromion, Superolateral clavicle
Insertion: Deltoid Tuberosity
Action: Flexion, Extension, & Abduction of arm
Innervation: Axillary n.
Teres minor
Origin: Lateral border of scapula
Insertion: Posterior/inferior aspect of greater tubercle (humerus)
Action: External Rotation of shoulder
Innervation: Axillary n.
Supraspinatus
Origin: supraspinous fossa
Insertion: Superior aspect of greater tubercle
Action: Abduction of arm
Innervation: Suprascapular n.
Infraspinatus
Origin: Infraspinous fossa

Insertion: Root of olecranon thru proximal shaft of Ulna
Action: Weak extension of elbow, active during supination/pronation of forearm
