- What are Integrated Care Plans (ICP)?
are similar to shared care plans in that they are developed by the entire integrated care team in consultation with the patient and family
- What is your understanding of Interprofessional Treatment Plans?
Treatment plans can be developed in response to any illness, but often refer to behavioral or addiction treatment where patients work with a central case manager (a health service coordinator could assume this position) plus a team of other professionals and programs.
- Explain Shared Decision Making and why it is so important in healthcare.
occurs when health professionals work with patients to help them make the best decisions about their own health and healthcare options
- What are team huddles?
can be described as daily mini-rounds where team members gather for a few minutes, usually at the beginning of a work day, to touch base and co-plan care for the day
- Define sociopolitical drivers.
are the conditions in which people are born, grow, live, work and age. They include factors like socioeconomic status, education, neighborhood, and physical environment, employment, and social support networks, as well as access to health care

The IDEA: Ethical Decision-Making Framework comprised of four steps and incorporates five conditions.
The four steps are:
