  • police intervention (section 28) = police apprehend person and take to a physician (usually in hospital) for examination who then determines if the patient is certifiable or not. If yes- then physician would complete a Form #4

    Form 10 is completed by the judge
    Form 9 would is completed by anyone with a reason to believe a person meets criteria for involuntary admission.
