Unit 1 Flashcards
What are the 7 vital signs?
- RR
- HR
- O2 Sats
- BP
- Temp (body)
- Exertion scale
- Pain Scale
Other vital signs
- Walking speed
2 Height = BMI - Weight = BMI
What is the HR for an Adult, N.born + 1 - 7yr
Adult: 60-100
N.Born: 100-130
1-7yr: 80-120

HR: When is it safe to mobilize?
> 40 +
What do you document for HR?
Quality + Quantity
- Strong Vs Weak

Normal for RR?
Eupnea
Adult: 12-18 breaths
Why do we measure RR?
SOB
What do we do when measure RR?
- Do not tell client you are counting.
- 30sec to 1 minute
What do we document from RR?
Quality of breathing + quantity breathes per minute
What are the two types of Blood Pressure?

What we document for BP?
- Record systolic + diastolic number’s.
- Time of day
- Meds
- Clients position
- Pre/post activity
What is a normal O2 Sat reading?
> 95% (usually 98-100),
*Adults + kids = 88%
When is safe to mob with O2 Sats?
- Over 88%
* Consider lower levels for COPD
What instrument do you use to measure O2 Stats
Oximeter
Normal body temp
37oC measure by thermometer
Fever body temp
38oC
How do we measure the quantity of pain?
- Numeric
- VAS: Visual Analogue Scale
- Pictures: Faces
Quality of pain is also assessed by…
- Location
- Change or remain static
- Description
What do we document for pain
Quantity + quality
How do we measure exertion?
- Borg: 6-20 pt scale. Cli perception
- Dyspnea: 4 pt scale. Cli perception+observer
- Talk Test: measure intensity
Measure Height + Weight
BMI = Mass (kg) / Height (m) BMI = Mass (lb) / Height (in) x703
