Preventive Health Assessment Steps

A
  1. The member is notified that he or she due for a PHA
  2. The member then completes an AF web Health Assessment
  3. Review of the member’s health record is conducted to identify medical issues, and all necessary clinical preventive services.
  4. CPS are provided , such as lab work , immunizations, tests, sepcialty services, and so forth.
  5. Member has a one on one encounter with the primary care manager (PCM) if necessary
