distribution of joints involved and presence of joint swelling, cutaneous, nail, and hair abnormalities (sclerodactyly, rash, alopecia) and their distribution, Raynaud phenomenon, tendon involvement, LAD and organomegaly, ocular involvement, pulmonary abnormalities, muscle weakness of limbs, head, neck, and pharynx, abdominal symptoms (hematochezia, diarrhea, and abdominal pain), recent travel, joint injury, or infections (STIs)
General lab tests to order with Inflammatory arthritis?
CBC, BMP, LFTs, CRP, ESR, Urine micro/Cx, CBC, serume ACE, CK
Autoantibody tests to order with inflammatory arthritis?
RF Anti CCP Ab ANA Ab to extractable nuclear antigens (Ro, La, Smith) ANCA
Imaging/tests in inflammatory arthritis?
Plain radiographs of hands, feet, sacroiliac joints, chest, US of affected joint(s), MRI of affected joints
PFTs, TTE, arthrocentesis with synovial fluid analysis (cell count with differential, microscopy, culture and sensitivities, crystal identification)
Synovial fluid analysis for inflammatory arthritis?
septic arthritis could be present with <50,000 cells/mm3 and a noninflammatory condition (such as osteoarthritis) could exist with a synovial fluid count >2000 cells/mm3
Treatment for inflammatory arthritis?
NSAIDS, Tylenol, tramadol, fish oil, topical NSAIDs
DMARDS in early RA or spondyloarthropathy
