UE Arthroplasty Flashcards
why is reverse shoulder arthroplasty potentially a good idea for someone with major rotator cuff damage? What else might be done if ER is an issue?
it increases the moment arm of deltoids.
Lats can be made to be an external rotator
what extra muscle is cut when doing a superior TSA approach as opposed to a deltopectoral?
anterior deltoid
what kind of instability is common following a TSA? what kind of motion must therefore be avoided?
posterior instability of the shoulder - avoid extension/adduction/internal rotation (hand behind back)
what are the general restrictions post TSA (any)
- no driving 3 wks
- no WB or heavy pushing/pulling on arm 6wks
- no lifting anything more than 3lbs for 12 weeks
- no ext past neutral 6-8wks
- no EOR hands behind back, abd+ER, or ER for 6 wks
- no AROM IR for 6 wks
- no RT for 6wks, for IR 9-12wks
what additional restrictions are present with a rTSA with LDTT?
How about just a rTSA?
what to avoid with superior approach?
- no lifting things heavier than 6lbs for 16wks
- no ROM at all 4-6 wks
- no RT 9-12 wks
- no ext past neutral 12wks
- no EOR hands behind back 12 wks
- no EOR IR 6wks
- no AROM flex 6wks
- no RT 12wks
how long is are TSAs/rTSAs immobilized?
TSA - sling 3-4wks
rTSA braced 4-6wks
what are the two types of elbow prostheses? how long immobilized after?
constrained (linked)
unconstrained (not linked)
usual in splint 4-8 wks but it’s removable
contraindications for TEA?
- no driving 6wks
- no WB on arm or push/pull 12wks
- no lifting anything more than 10-15lbs FOR LIFE
- no repetitive throwing for LIFE
- no elbow ext against grav for 6 wks
- no ext+sup 6wks post non-constrained only
- no elbow flexion or wrist RT for 6-8 wks
- no elbow extension RT for 12wks
what are some activities not recommended after TSA?
hockey
gymnastics
football
rock climbing
