Focus Assessment
Status determination of a specific problem identified during the previous assessment
Ongoing process, integrated with nursing care, few minutes to a few hours between assessments
Admission Assessment
Initial identification of normal function, functional status, and collection of data concerning actual or potential dysfunction
Baseline for reference and future comparison
Done within specified time frame
Time-lapse Reassassment
Comparison of patient’s current status to baseline obtained previously; detection of changes in all functional health patterns after extended periods of time has passed
Several months (3, 6, or 9) between assessments
Emergency Assessment
Identification of life-threatening situation
Done at any time a physiologic, psychological, or emotional crisis occurs
