What is the glycaemic target on diabetic drugs?
- 48 mmol HBA1C if on drug non associated with hypos
2. 53 mmol if on drugs associated with hypos
What is the first line tx?
- Metformin
offered if HB1AC rises to 48 on life style interventions
What is second line tx?
- If HB1AC rises to 58 on Metformin
- Add either
- sulfonylurea
- gliptin
- glitazone
- SGLT - 2 inhibitor
‘DUAL THERAPY’
What is third line tx?
- TRIPLE THERAPY
OR
- Consider insulin therapy alone
What is the criteria for GLP-1 to be started?
- Given as ‘ triple therapy’ with; metformin/sufonylurea/+ GLP-1
- BMI > 35
or
- BMI < 35 but insulin therapy would have significant ocupational implications
What is assessed at GLP-1 follow up?
reasses in 6 months and only continue if
- reduction of atleast 11 mmol
- weight loss of 3% body weight
What is the mx if metformin in CI?
- use sulfonylrea/gliptin/pioglitazone
