A
- Avoid trauma
- Use good supportive bra
- OTC analgesics
- Encourage breast self exam and regular f/u

- Watch and f/u
- Removal if obstructive or suspicious

- Wide excision if nonmalignant
- Simple mastectomy = controversial tx of malignant form

- Refer to surgery if necessary –> removal of affected duct

- Total duct excision (resolves discharge, allows nipple eversion)

- Self-limiting
- R/o malignancy or carcinoma if not resolved in 2 weeks (surgical bx)

- Staph, MRSA tx

- May need abx
- Staph: Penicillin, Dicloxacillin, Amoxicillin, Augmentin, Cephalexin, Doxycycline, TMP-SMX
- MRSA: Doxycycline, TMP-SMX
- I&D if abscess

- Rest
- Anti-inflammatories
- Warm compresses
- Well-fitting bra
- Hydration

- Early stage - abx, continue breastfeeding
- Later stage - abx, I&D, d/c breastfeeding
- Non-lactating - I&D
- Possible excision if duct involvement

- Lumpectomy + radiation
- Masectomy
- ER (+): hormone therapy

- Surgery: lumpectomy or mastectomy

Incurable
- Palliative radiation and chemotherapy. No surgery.
