Who further torments Malvolio and how?

Then he leaves and returns as Feste. Feste alternates between Sir Topas’ and his own voice to torment Malvolio further.

No, Malvolio is not really insane. Maria, Sir Toby and the others are just tormenting him to make Malvolio believe he is mad. All Malvolio did was follow the instruction in the letter that Maria wrote in Olivia’s perspective to make him appear insane.
Then Sir Toby decided to lock him in a dark room to torment him more.

There is a lot of confusion about Cesario and Sebastian in the begging of Act 5. The Duke, Antonio, Olivia, Sir Toby and all of the other characters in this act mistake Cesario for Sebastian and Sebastian for Cesario. They are all in shock because they had no idea that Cesario and Sebastian had a twin. In the beginning of this act, Cesario and Sebastian are also very confused because they thought each other had died.

Olivia reveals that Maria actually wrote the letter by saying “Alas, Malvolio, this isn’t my handwriting, though I admit it’s very like it. Beyond a doubt it’s Maria’s.”

When the Duke finds out that Cesario is actually a woman, he is astonished and realizes that Cesario always did say that he would never love a woman as much as he loves him. On that note, the Duke asks Viola for her hand in marriage and to see her in woman’s attire. He says that as a woman, Viola will be his wife and queen of his love.
How does the play conclude?
The play concludes with Feste singing. Viola and Duke are now to be married, Malvolio is said not to be mad but only fooled by Maria and Olivia and Sebastian are married.
