What is the concept of a personalised approach to care?

A

Giving the patient the information he needs to make the decision, showing them their risks, assessing where they are on cycle of change - think of requiring versus needing.

They have choice and control over how their care is planned and delivered.

What is shared decision making and why is it important?

A

Clinicians and patients share the best available evidence when faced with the task of making decisions and where patients are supported to consider options, to achieve informed preferences

More personal approach, patient feels in control

How is variation a challenge in practice and outcomes?

A

Patients are complex and so guidelines on particular conditions may cause variations when patient is co-morbid.

This is a challenge as it makes it more difficult to ensure the patient is being treated in the best way possible for they themselves.

Why is there a need to reduce harm and waste?

A

To prevent overdiagnosis and putting people in a position where the key life decisions they make are based on disease they may not even have - causing harm

Some medicine may not even have an effect (that is desirable) on certain people thus being wasted

A

In order to reduce injury to patients, staff etc through reduction of medical errors and safety netting

A

To ensure work is of good quality and that any danger to patients or staff are being reduced, and that the best methods are being used within the healthcare system
