Initial presentation of TB
Will initially present as pnuemonia, but some of the caili will survive and by walled off by the immune system (Ghon Lesions)
TB Diagnostic Tests
Mantoux, actid fast satin, sputum culture, QuantiFERON (gold standard)
Mantoux Tuberculin Test
Wheal <5 mm = Negative
Wheal 5-9 mm= Consider suspicious in non-high-risk categories
*With high risk categories (exposure, ghon lesions, HIV, under age 5) it is considered a positive result
Wheal 10 mm of greater=Positive
QuentiFERON
QuentiFERON is a blood test that will give results after 24 hours, but unable to distinguish between active vs latent TB infectiosn
Primary Bacteria that causes TB
mycobacterium tuberculosis
TB chest x-ray
Increased opacity, ghon nodule, cavities, effusion, calcificiation and fibrosis
TB PFT
Restrictive presentation
TB Hemodyanmics
Pulmonary HTN
Right sided heart failure
TB Pharmacology
Isoniazid is the most effect first line anti TB agent
Rifampin bactericidal and communly used with isoniazid
Ghon Lesions
Ghon Lesion is the only abnormal thing you would see in latent TB and may not always be there
