What is tuberculosis?
TB is a communicable infectious disease, transmitted almost exclusively by cough aerosol, and characterised pathologically by caseating necrotising granulomatous inflammation
Risk factors for TB?
Poverty Malnutrition Overcrowding, HIV/immunosuppression Underlying chronic disease (COPD) Poor control programmes/breakdown of health services Alcohol/IV drug abuse
Symptoms of TB?
Cough Haemoptysis Fever, night sweats Weight loss Loss of appetite
Investigations for TB?
CXR
Sputum
Histology from biopsy
What is primary TB?
Infection with TB with no pre-existing immunity
What is secondary (post-primary) TB?
Due to reactivation of old lesions/gradual progression of primary TB into chronic form
Primary TB signs on CXR?
Upper zone opacities
Secondary TB signs on CXR?
Cavitation
Treatment of TB?
RIPE
Rifampicin
Isoniazid
Pyrazinamide
Ethambutol
Side effect of rifampicin?
Reddish colouration of urine, sweat, saliva and tears
Side effect of ethambutol?
Optic neuritis
Side effect of pyrazinamide?
Hepatitis and arthralgia
Side effect of isoniazid?
Hepatitis and peripheral neuropathy
