Tricuspid regurgitation Flashcards
Define tricuspid regurgitation
Backflow of blood from right ventricle to right atrium during systole
Aetiology of tricuspid regurgitation
5 groups
Congenital Functional Rheumatic heart disease Infective endocarditis Other
Aetiology of tricuspid regurgitation - congenital
2
Ebstein’s anomaly (malpositioned tricuspid valve)
Cleft valve in osmium primum
Aetiology of tricuspid regurgitation - functional
2
Consequence of right ventricular dilatation (e.g. due to pulmonary hypertension)
Valve prolapse
Aetiology of tricuspid regurgitation - other
4
Carcinoid syndrome
Trauma
Cirrhosis
Iatrogenic
Epidemiology of tricuspid regurgitation
general, type
Differs based on cause
Infective endocarditis is MOST COMMON cause
Presenting symptoms of tricuspid regurgitation
9
Fatigue Breathlessness Palpitations Headaches Nausea Anorexia Epigastric pain made worse by exercising Jaundice Lower limb swelling
Signs of tricuspid regurgitation on physical examination - pulse
Irregularly irregular if AF
Signs of tricuspid regurgitation on physical examination - inspection
(3)
Raised JVP w/ giant V waves (which may oscillate to earlobes)
Caused by transmission of high right ventricular pressure into great veins
Giant A waves may also be present
Signs of tricuspid regurgitation on physical examination - palpation
Parasternal heave
Signs of tricuspid regurgitation on physical examination - auscultation
(2)
Pansystolic murmur
heard best at lower left sternal edge
louder on inspiration (Carvallo sign)
Loud P2 component of 2nd heart sound
Signs of tricuspid regurgitation on physical examination - chest exam
(2)
Pleural effusion
Causes of pulmonary hypertension
Signs of tricuspid regurgitation on physical examination - abdo exam
(2)
Palpable liver (tender, smooth, pulsatile) Ascites
Signs of tricuspid regurgitation on physical examination - legs
Pitting oedema
Investigations for tricuspid regurgitation
5 groups
Bloods ECG CXR Echocardiography Right heart catheterisation
