What are symptoms of Hyperthyroidism?
- Anxiety
- Exophthalmos (bulging eyes)
- Goitre
- Tachycardia, Palpitations, Atrial fibrillation
- weight loss
- tremor
- diarrhoea
What is Graves Disease?
= autoimmune condition where Abs bind to TSH receptor and cause excess thyroid hormone prod.
- form of Hyperthyroidism
Diagnosis of Hyperthyroidism
- Low TSH
- High serum T4
How to differentiate bw Primary and Secondary Hyperthyroidism
Primary:
- low TRH
- low TSH
- High T3 and T4
Secondary:
- low TRH
- higher TSH
- High T3 and T4
Treatment for Graves’ Disease
Thiaonamides:
1. Carbimazole (first-line treatment)
2. Propylthiouracil
What do Thionamides do?
- inhibit iodination of Tyrosine on thyroglobulin
- decrease T3/T4 synthesis
AEs of Thionamides
- can cross the placenta -> causing congenital hypothyroidism
- Rashes
- Headaches, nausea, jaundice
What does excess iodide do
- although iodide needed for T3/T4, excess iodide can inhibit release of T3/T4
What are the clinical presentations of Hypothyroidism
- weight gain
- slowing of activities
What happens to TRH, TSH and T3/T4 in HYpothyroidism?
- increased TRH
- increased TSH
- decreased T3 and T4
Treatment for Hypothyroidism
T4 - used orally
T3 - used intravenously
Dietary iodine
