Require > 3 medications to control BP

Rx approach:
A
- Optimise diuretic
- Optimise ACE-I w/ CCB
- Add mineralocorticoid antagonist

How to optimise diuretic?
A
- Switch HCTZ to Chlorthalidone - x2 more potent
- eGFR < 30 (renal impairment) - switch to loop diuretic

Cautions with spironolcatone:
A
- Risk of HyperK when used with - ACE-I or NSAIDs or CKD patients
- CI if eGFR < 30
