What is the general first line anti-HPT agent?

For patients with diabetes (with NO proteinuria), your first-line options will be:
- Thiazides - HCTZ
Same as general population
If your DM patient has albuminuria, you may consider starting with?
ACE-I or ARB
These drugs have been shown to slow the progression of albuminuria and help preserve kidney function.
1st line Rx for CKD pt’s especially with proteinuria:
ACE-I or ARB
1st line Rx for Black pt’s:
- Thiazide
- Or CCB
Black patients with diabetic nephropathy, you’ll still get the best response by using?
ACE-I or ARB first
1st line Rx for Pregnant pt’s?
- Methyldopa (alpha-2 agonist)
- Or CCB - Nifedipine
- Or B-blocker - Labetalol
For patients with a recent myocardial infarction or heart failure - 1st and 2nd line options?
1st line: B-blocker
2nd line: ACE-I or ARB
For patients with stable coronary artery disease - 1st and 2nd line options?
1st line: ACE-I or ARB
2nd line: B-blocker or CCB
