Tension pneumothorax: S/Sx
*Clinical Dx not by CXR
Resp. distress, shock, distended neck veins, unilateral decrease in breath sounds, hyperresonance, cyanosis (late sign)
Flail chest and pulmonary contusion: management
O2, reexpand lung, intubate as indicated, judicious fluids, analgesia
Massive hemothorax: Definition, S/Sx
Systemic/pulmonary vessel disruption
> 1500 mL blood loss; flat v distended neck veins; shock w/ no breath sounds and/or percussion dullness
Indications for laparotomy (blunt trauma)
Hemodynamically abnormal w/ suspected abdominal injury (FAST)
Free air; diaphragmatic rupture; peritonitis; positive CT
Pelvic fx assessment
Leg length discrepancy, external rotation
Rectal/GU/vaginal exam
Palpation of pelvic ring, stability
ICP ranges: normal v. abnormal v. severe
Normal = 10 mm Hg
Abnormal > 20 mm Hg
Severe > 40 mm HG
