trauma and stressor related disorders
exposure to a traumatic or stressful event is diagnostic criteria
Reactive Attachment disorder
in children: inhibited, emotionally withdrawn behavior towards caregivers.
- rarely or minimally seeks comfort when distressed
- and rarely/ responds to comfort when distressed
symptoms:
- Withdrawn
- Neg affect, lack of pos emotions
- Unable to be comforted
- display neg emotions of fear, sadness, irratibility
comes from:
- neglect
- repeated changes in caregivers
must be atlead 9 months old, because thats when kids start to form selective attachments
Disinhibited Social Engagement Disorder (DSED)
In children: overly approaches and interacts with unfamiliar adult
- they don’t check in with adult after venturing away
- willingness to go off with a streager
comes from:
- social neglect
- changes in caregivers
- unusual rearing settings
must be atleast 9 months
PTSD
development of symptoms following exposure to traumatic event(s)
- can be fear-based rexperienceing, emotion adn behavioral symptoms
- can be anhedonic or dysphoric mood states and neg conditions
- can be arousal and reactive-externalizing symptoms
can be with depersonalization and derealization
symptoms must last over a month
treatment: CBT, exposure, emdr
bio psycho social ptsd
-Psychological: Easy burnout, fatigue (lots of energy is spent on hypervigilance)
-Social: Isolation, persecution, especially if someone with PTSD blames people in their lives (or
themselves) for onset
-Biological: Disturbed sleep, difficulty keeping up with school and work. Possible weight gain or
loss due to stress
Acute Stress Disorder
Similar to PTSD, but minimum onset after 3 days from traumatic event, maximum duration 1 month.
Past 1 month, passes into PTSD threshold
- Similar to PTSD
- Intrusion symptoms
- Negative mood
- Avoidance symptoms
- Dissociative symptoms
- Sleep disturbance
- Alterations in arousal level
Prolonged Grief Disorder
It is a distinctive persistent grief that occurs since the death of an individual with whom the
client had a close relationship
- In adults: Symptoms last over a year
Impact on Functioning
- Disruption in one’s own identity
- Feeling that life is meaningless
- difficulty connecting interpesonally
- Intense emotional pain
- Belief that the death never occurred
- Avoiding reminders of their close ones death
What Clinicians Need to Know
- Differential Diagnosis (vs PTSD): Triggering event must be death of a loved one
- An associated feature is that one may feel guilt about the death even if they had no
involvement
treatments:
- CBT, CBT w exposure therapy, prolonged grief therapy
Adjustment disorder
Characterized by over-exaggerated reaction to identifiable stressor
- marked distress out of proportion to severity or intensity of stressor
- significant impairment in social/occupational/other areas of functioning
- Often preliminary diagnoses for insurance purposes
- Symptoms must be resolved w/in 6 months
can be with depressed mood, anxiety, both, disturbance of conduct, mixed disturbance of mood and conduct
