Trauma Alert (1 Criteria)
Airway
Patient receives active airway assistance beyond the administration of O2.
Trauma Alert (1 Criteria)
Circulation
Patient lacks a radial pulse with sustained HR greater than 120 bpm.
Systolic BP less than 90 mmHg
Trauma Alert (1 Criteria)
BMR
Patient exhibits a score of four or less on the motor assessment of the GCS.
Trauma Alert (1 Criteria)
Spinal
There is presence of paralysis, or loss of sensation, or there is a suspicion of spinal cord injury.
Adult Trauma Alert (1 Criteria)
Cutaneous
Patient has 2nd or 3rd degree burns to 15% or more of total body surface area.
Amputation proximal to wrist or ankle
Penetrating injury to head, neck and/or torso
Trauma Alert (1 Criteria)
Long Bone Fx
Patient reveals signs or symptoms of two or more long bone fracture sites
Trauma Alert (2 Criteria)
Airway
Patient has a RR of 30 or greater
Trauma Alert (2 Criteria)
Circulation
Patient has a sustained HR or 120 BPM or greater

Circulation
Patient has sustained HR of 120 BPM or greater
Trauma Alert (2 Criteria)
BMR
Patient has a BMR of 5 on the motor component of the GCS
Trauma Alert (2 Criteria)
Cutaneous
Patient has soft tissue loss from either a major devolving injury or major flap avulsion greater than 5 inches
Sustained GSW to extremity
Trauma Alert (2 Criteria)
Long Bone Fx
Patient has single long bone Fx
Fall from 10 feet or greater
Trauma Alert (2 Criteria)
Age
Patient is 55 year of age or older
Trauma Alert (2 Criteria)
Mechanism of Injury
Ejection from Motor vehicle
Driver impacted steering wheel causing deformity
If the Patient’s GCS score is ______ or less, the Patient shall be considered a trauma alert patient.
