trauma Flashcards
what is the most important step in a trauma case
triage
most common type of shock seen in poly trauma
hemorrhagic shock
why is hemorrhagic shock an issue
lack of oxygen delivery to the tissue from decreased CO and MAP
what happens to tissues and cells in hemorrhagic shock
increased oxygen radicals and metabolites, increased blood lactate from anaerobic metabolism
what do you assess in your polytrauma patient first
CO-pulses
CRT, HR, temp
if you cant palpate dorsal pedal pulses what is the BP
<60mmHg
what is flail chest
indicative of >3 rib fractures
if the patient is dyspneic what to do
flow by O2 support. ascult and perform TFAST
what is the order to assess body systems
cardiac, respiratory, neurological, urinary, skeletal/wound- last
normal urine output
1mL/kg/hr
patient comes in and is hemodynamically unstable. what do you do
place a catheter, pull blood, bolus fluids, flow by O2, +- sedation, recheck cv status
what should you do if the patient is respiratory and CV unstable
afast and tfast
what is our go to fluid to bolus
LRS
what can you consider giving if the patient is having ongoing hemorrhage and its not a dilutional or consumptive coagulopathy
epsilon aminocaproic acid
