RFs: spinal precautions
- fall from>1m or >5 stairs
- axial lod to head (diving)
- high speet vehicle collision/rollover acident
- ejection from vehicle
- bike collision
- horse riding accidents
indications of full inline spinal immobilisation
- significant distracting injuries
- under influence of drugs/alcohol
- confused/uncooperative
- reduced level of consciousness
- spinal pain
- hand or foot weakness
- altered sensation hands or feet
- priapism
- hx spinal problems
- spinal assessment not done @ scene
high risk RFs: cervical spine injury
- 65+ y/o
- dangerous mechanism of injury
- cervical spine tenderness
- focal peripheral neuro defecit
- paraesthesia of limbs
- patient with head injury
- GCS <=13
logroll procedure
INTRODUCTION
* Gather team
* 5 trained people
* Designate tasks
* Leader takes head position
* 3 people along the side of patient (biggest at patients chest)
* 1 person will be designated to carry the indication for logroll
* Washes hands
* Dons gloves + apron +/- mask and visor (during COVID)
* Gathers equipment if needed: e.g. scoop, clean sheets, KY jelly for PR
* Closes curtain and sides of trolley down
- Introduces themselves to patient & confirms patient details
- Explains procedure to patient and gains consent
- Leader talks through the plan for the log roll with team
- Leader starts manual in line immobilisation and tapes / blocks are removed
- Logroll commenced on leaders command (READY STEADY ROLL and go on ROLL)
- Indication carried out: e.g. back exam, PR , sheet change, wet clothes off
- Patient rolled back to supine position on leaders command
- Blocks and taped reapplied
- Patients dignity preserved (blanket)
- Sides of trolley pulled back up
