Hyper-resonant percussion
Tension pneumothorax
Most common cause of tension pneumothorax
Mechanical ventilation in patients with pleural injury
Multiple rib fractures resulting in infrequent chest motion

Pathogenesis of Flail Chest
Chest wall disconnects from thoracic cage due to multiple rib fractures
Management of simple pneumothorax
Chest drain - do not aspirate in traumatic pneumothorax for risk of tension
Management of Haemothorax
<1.5L or 200ml/hour - wide bore 36F chest drain
> 1.5L or >200ml/hour for two hours - thoracotomy
Causes of haemothorax
laceration of lung or intercostal vessel, or internal mammary artery
What is Beck’s Triad?
Elevated venous pressure, reduced arterial pressure and reduced heart sounds = cardiac tamponade
Muffled heart sounds and pulsus paradoxus
Cardiac tamponade
Widened mediastinum and contained haemoatoma
Aortic disruption
RTA -> indistinct left hemidiaphragm
Diaphragm disruption
Commonest abdominal injury caused by blunt trauma requiring laparotomy
Splenic injury
Blood at the urethral meatus
Urethral tear
High riding prostate
Urethral disruption
