What is transient synovitis?
A
- = “irritable hip”
- Benign, self-limiting cause of hip pain due to inflammation of hip joint synovium

Epidemiology:
- How common is it?
- age group
- M/F?
A
- Most common cause of non-traumatic hip pain and limp in pre-school age group
- Age 3 - 10 years
- M > F (2:1)

What is the usually precipitant?
A
URTI/pharyngitis/OM

What kind of pain is seen in transient synovitis?
A
- Sudden onset pain and limp
- Groin pain usually, sometimes referred to thigh/knee

What kind of signs are seen with transient synovitis?
A
- Antalgic gait: hip abducted and ER
- Still capable of moving hip through ROM

What must you remember when diagnosing transient synovitis?
A
Dx of exclusion only - overlaps with septic arthritis!

What kind of Mx is there for transient synovitis?
A
- Conservative - symptomatic and anti-inflammatory medications, rest with return to full activity as tolerated
- Recover after one week or so
- 5-10% have second episode
