What is it?
A
- Type of thrombotic microangiopathy (TTA) that is toxin-mediated

Aetiology of HUS
A
- Very complex, most usually Shiga toxin from E.coli as part of an infectious colitis

Clinical features of HUS
A
Characteristically extremely sick and at risk of rapid decompensation with multi-organ involvement including some/ all of:
- AKI- oliguric, hyperkalaemic and hypertensive complications common
- Neurological- irritability, seizures, decreased conscious state
- Bleeding risk with severe thrombocytopaenia and anaemia, INR is normal, fibrinogen raised (c.f. DIC)
- Cardiac- hypertensive cardiomyopathy/ myocarditis/ ischaemia
- Gastrointestinal- bloody diarrhoea/ perforation
- Pancreatitis
- Pulmonary- oedema, haemorrhage

Ix for HUS
A
- Blood film must show fragmented RBCs for Dx
* DCT negative (as destruction is mechanical as opposed to immune-mediated)
