Aetiology of HUS

A
  • Very complex, most usually Shiga toxin from E.coli as part of an infectious colitis

Clinical features of HUS

A

Characteristically extremely sick and at risk of rapid decompensation with multi-organ involvement including some/ all of:

  1. AKI- oliguric, hyperkalaemic and hypertensive complications common
  2. Neurological- irritability, seizures, decreased conscious state
  3. Bleeding risk with severe thrombocytopaenia and anaemia, INR is normal, fibrinogen raised (c.f. DIC)
  4. Cardiac- hypertensive cardiomyopathy/ myocarditis/ ischaemia
  5. Gastrointestinal- bloody diarrhoea/ perforation
  6. Pancreatitis
  7. Pulmonary- oedema, haemorrhage

Ix for HUS

A
  • Blood film must show fragmented RBCs for Dx

* DCT negative (as destruction is mechanical as opposed to immune-mediated)
