Transfusion medicine 2 Flashcards
What does a low speen spin get you?
Packed Red cells and platelet rich plasma
What do you put through a high speed spin
Platelet rich plasma…generates plasma and a platelet pellet
The plasma and platelet button are then recombined and this is called
Random donor platelett unit
Do plateletts express ABO antigens
Yes
What are two things to keep in mind regarding the expression of ABO antibodies by plateletts?
1.) Plateletts are cleared faster if they are ABO incompatible
2.) Can induce hemolysis
One other thing: OPTIMAL PLATELETT TRANSFUSION IS WITH IDENTICAL ABO platelets.
What are the risks of the differential centrifugation method?
Contamination
Damage to platelets by filtration
Damages to platelets by apheresis method?
Exposes recipient to about 300ml of plasma from one donor
How low does platelet count have to get before GI bleeding occurs?
Starts around 7,000/ul….so around 7-5
What platelet number should you transfuse at prior to surgery
50,000 and below
What platelett count should you transfuse at for active bleeders
50,000 and below
What should you transfuse at before neurosurgery
75,000
Before most invasive procedures, transfuse platelets if less than what?
50k
For a normal platelet count, how many platelets will someone make in 24 hours?
15,000…for people on drugs which inhibit platelet function that are bleeding internally, standard practice has been to transfuse. This may not be correct
The secret to determining compatability when in comes to platelett transfusion
Look at the donor’s blood and the antibodies they have. If they don’t have an antibody to the recipients blood, you’re good.
5 units of FFP is what percent of blood volume?
22.5 %
