Complete Independence:
If walking: pt safely approaches, sits down is a chair, transitions to standing. Pt also transfers safely form bed to chair.
If w/c: pt wheels to a bed or chair, lockes brakes, lifts foot rests, removes arm rest if necessary, and performs either a standing pivot or sliding transfer (no board) and returns. Performed safely.

Modified Independence–pt requires adaptive or assistive device aka sliding board, lift, grab bars, or special seat/ chair/ brace/ crutches: or longer than a reasonable amount of time, or safety considerations. (prosthesis or orthosis is a/d for this transfer.

Supervision/Setup–(SBA, cueing, coaxing) or (positioning slide board, moving foot rests, etc.)

Min Contact A – Only touching and pt performs 75% or more. PT 25% or less.

Mod A – Pt requires more help than touching, 50-75%. PT 25%-50%.

Max A – Pt performs 25-50%. PT 50-75%.

Total assist – Pt <25%. PT >75%

Activity Does Not Occur – Enter code 0 only for the admission assessment. Pt does not transfer on their own . and PT does not transfer pt or use a lift device. Use should be rare.
What position must the patient begin and end in, in bed to chair transfers?
Supine.
