Define 7
Complete Independence–The pt safely performs all tasks described within a reasonable amount of time, without modification, assistive devices or aids.
Define 6
Modified Independence–1 or more may be true: requires assistive device or aid, takes more that a reasonable amount of time, or there are safety/risk considerations.
Define Modified Dependence
Patient performs 50% or more of effort.
Define Complete Dependence
Patient performs <50% of effort.
Define 5
Supervision/Setup–Patient requires only SBA, cueing, or coaxing, no physical contact; or the helper sets up the environment or orthoses/assistive devices.
Define 4
Minimal Contact Assistance–Patient only requires touching and performs 75% or more of effort.
Define 3
Moderate Assistance–Patient requires more help than just touching…patient performs between 50 & 74%. PT between 25 & 50%.
Define 2
Maximal Assistance–Patient expends between 25 to 49% of effort. PT 50 & 75%.
Define 1
Total Asssistance–Patient expends < 25%.
Define 0
Activity does not occur–Patient does not do it, and the PT cannot do it for them. Ask to see if someone else has seen the patient do this.
