Why are some reasons why a patient would be transferred?

A
  • patient’s condition may changed and they need to be moved to a unit where their needs can be better met
  • patient may need a private room for infection control or isolation
  • patient can request to be moved if and when a bed becomes available

What is the 14 step process to for a patient transferring into your unit?

A
  1. Bed Control will notify unit of patient changes and new bed assignment
  2. Unit transferring the patient to your unit will call to arrange a convenient time.
  3. Record on Daily Activity Sheet / White Board.
  4. Introduce yourself to patient upon arrival on the unit.
  5. Notify assigned nurse.
  6. Place addressograph in holder/or labels at front of chart.
  7. File MAR in Medication Binder
  8. Make necessary changes to Kardex and place in proper location.
  9. Complete/print out new labels for patient’s chart, HOB, door, etc.
  10. Notify Dietary prn.
  11. Check patient’s chart for any new or incomplete doctor’s orders. Transcribe orders prn and place chart in correct location in chart rack.
  12. Place old chart in correct location.
  13. File all miscellaneous paperwork in the correct place. This may include diagnostic tests that were ordered prior to patient’s arrival on your unit, and requisitions for specimens to be collected (urine, stool, etc.)
  14. Advise Physician prn.

What are the 12 steps for transferring a patient from your unit to another unit?

A
  1. Bed Control will notify unit of patient changes and new bed assignment
  2. Call receiving unit to arrange a convenient time. RN will give report to receiving RN.
  3. Arrange for Porter to transport patient.
  4. Record on Daily Activity Sheet / White Board.
  5. Ensure all orders are transcribed.
  6. Collect together the Addressograph prn, Kardex, chart, old chart, forms from Nurse’s Clipboard, MAR from Medication Binder, and all relevant paperwork and send with patient.
  7. RN will gather patient’s personal belongings.
  8. Transfer patient in computer where required.
  9. Notify Housekeeping.
  10. Notify Dietary prn.
  11. Remove labels from door, HOB, med cart, etc.
  12. Advise Physician prn.

What are the 11 steps for an intra transfer?

A
  1. Notify Bed Control of the bed change/enter changes in computer where applicable.
  2. Record on Daily Activity Sheet / White Board.
  3. Place Addressograph in new slot in holder prn.
  4. Place Kardex in correct section of Kardex holder.
  5. MAR – move to correct section of the Medication Binder.
  6. Change labels on chart, HOB, door, med cart, etc.
  7. Ensure any reports still to be filed are filed correctly with respect to the new room number .
  8. Put chart in correct slot in chart rack.
  9. Notify Dietary prn (if near a meal call them).
  10. Notify Housekeeping.
  11. Advise Physician prn.
