How does Tracheobronchial tree injury (TBTI) usually occur?
A
- Severe blunt trauma
- Penetrating trauma to the chest or neck (>1/2 due to MVA)

Physical examination (signs) of those with TBTI. (5)
A
- Respiratory distress
- Hemoptysis, cough
- Subcutaneous emphysema
- Wound opens into pleural space –> large pneumothorax with persistent air leak
- Pneumomediastinum on Chest X-ray

What is the treatment of TBTI?
A
- May need endotracheal intubation
-With bronchoscope - Multiple chest tube insertion required
- Continues to bubble vigorously with little resolution of pneumothorax
- Surgical repair: Thoracotomy

- De novo generation or infiltration of air underneath the dermal layers of skin
