Fetal fibronectin - to test protein that adheres amniotic sac to uterus Cervical dilation Contractions Speculum exam - HVS/LVS TVUS CTG
PPROM tests
History
Speculum exam - sterile!! Pool of fluid in vagina/posterior forbid
TVUS
Nitrazine test - pH detection, the more blue the more likely ROM
Fern test - ferning crystalline pattern of fried amniotic fluid
In suspected labour/already in first first stage, always ask:
Vitals of mother
CTG for baby
Cervix dilatation and contractions to give an idea of when it’ll happen
Prolonged first stage of labour
More than about 20 hours in primigravida (<0.5cm/hr over 4 hours)
More than 14 hours in a multigravida (<0.5cm/hr or slowing of progress)
Prolonged second stage of labour
> 2 hours in primigravida
>1 hours in multigravida
Prolonged third stage of labour
> 30 mins
Questions to ask for TPL/PROM/Labour
Gestation and parity ROM? Cervix dilatation Contractions Pain or bleeding Mum - obs, ANC Bub - CTG
