isopropyl alcohol (isopropanol) ingestion

A

alcohol dehydrogenase converts isopropanol to acetone (a ketone), which does not require Bicarb(therefore no metabolic acidosis).

  • osmolal gap: lab - calc
  • calc: 2xNa + BUN/2.8 + Glucose/18
  • normal gap: 10
  • if increased, there are unmeasured osms (ethanol, methanol, isopropyl alcohol, ethylene glycol)
  • methanol (met acidosis, retinal abnorm)
  • ethylene glycol (met acidosis, kidney failure)
  • hypokalemia (toluene)
