Acetaminophen Toxicity (Tylenol) symptoms
Stage 1: Malaise, RUQ pain (0-24 hours)
Stage 2: Elevated liver labs (18-72 hrs)
Stage 3: Maximal liver injury (72-96hrs)
Stage 4: Recovery (4-21 days)
Cure for Acetaminophen Toxicity
N-Acetylcysteine
Binds with toxic metabolites of acetaminophen
Given within 8-10 hrs
Acetylsalicylic acid toxicity (Aspirin)
No antidote
Treatment:
ICU: Sodium bicarbonate
Hemodialysis
Beta-blocker cause hyper/hypoglycemia?
Hypoglycemia
Calcium Channel blocker cause hyper/hypoglycemia?
Hyperglycemia
Flumazenil
Competitive antagonist for benzo OF
May cause withdrawals (concerns for seizures)
Toxic Alcohol poisoning
Hallmark sign is WIDE GAP METABOLIC ACIDOSIS
Methanol
Wood alcohol
Ethylene Glycol
Antifreeze
Calcium oxalate crystals found in brain, heart, and lung tissues
Isopropanol
Rubbing Alcohol
Direct effect on Organ tissues
Treatment of Toxic Alcohols
Administration of ethanol alcohol or Fomepizole
Beer Potomania
Dilutional hyponatremia from drinking too much beer
TCA OD
Wide QRS
Hyperthermia with impaired sweating
Hypotension
Digitalis poisoning
GI distress is usually first sign
May see green or yellow halos
EKG changes:
-Slowing heart rate
- ST depression “ice cream scoop”
