Chlorhexidine indications and dosing

1.) Pain: Adults, 15-60 mg PO Q4H prn, with APAP 300-1000 mg
Colchicine indications and dosing
1.) Gout, acute: 1.2 mg PO at the first sign of flare followed by 0.6 mg 1 hour later, max 1.8 mg over 1 hour
2.) Gout, prophylaxis: 0.6 mg PO QD to BID, max of 1.2 mg/day or onset of diarrhea
3.) Familial Mediterranean fever: Children 4-6 yo, 0.3-1.8 mg daily, children 6-12 yo: 0.9-1.8 mg QD, Children >12 y and adults: 1.2-2.5 P OQD, increase or decrease by 0.3mg/day
Colesevelam indications and dosing
1.) Primary and familial hyperlipidemia: 1.875 mg BID or 3.75 g PO QD
2.) Diabetes Mellitus, type 2: 1.875 mg PO BID or 3.75 mg PO QD
Conjugated Estrogens indications and dosing
1.) Abnormal vasomotor function (menopause), atrophy of vagina or vulva, postmenopausal osteoporosis prophylaxis, female hypogonadism syndrome: 0.3 mg PO QD, continuously or cyclically
2.) Primary ovarian failure: 1.25 mg PO QD cyclically (3 weeks on, 1 week off); adjust dose to individual response
Crisaborole indications and dosing
1.) Atopic dermatitis: adults and infants >3mo age, apply a thin layer to affected area BID
