Toothwear Part 2 Flashcards
1
Q
Lack of posterior support
A
- modifying factor of toothwear
- increases severity of wear
- increases rate of progression of wear
- can lead to occlusal collapse
- functional & aesthetic problems
2
Q
Why avoid complete dentures in toothwear pts?
A
- bruxism doesnt stop
- cause fractured dentures, ridge resorption, pain & ulceration under complete dentures
3
Q
Removable prosthodontics for toothwear pts
A
- overdentures
- transitional dentures
- metal based dentures
- simplifying small saddles
- rehabilitation -> involves increase in vertical dimension of occlusion (OVD)
4
Q
What is an overdenture?
A
- any removable prosthesis that rests on one/ more remaining natural teeth, roots of natural teeth/ dental implants
can be called overlay denture/ overlay prosthesis
5
Q
Advantages of overdentures?
A
- correction of occlusion and aesthetics
- support
- toothwear management
- preservation of ridge form
- proprioception
- denture retention
- can be used with precision attachments
- when extractions need to be avoided in MRONJ and radiotherapy pts
- psychological benefits
- useful in elderly patients
- eases transition to edentulism
6
Q
Disadvantages of overdentures
A
- need for good oral health
- increased caries/ perio problems
- care homes
- denture fracture
- discomfort/ infection
- medical history
- potentially more traumatic extractions
7
Q
How to care for overdentures?
A
- good oral hygiene
- fluoride toothpaste application to roots
- regular examinations & radiographs
- denture hygiene
8
Q
What does transitional dentures do?
A
- increase OVD in cases where poor posterior support to create space for restorations
- wear them for a few months and if copes with transitional dentures and increased OVD, then:
- get ride of impossible
- crowns with rest seats and undercuts
- definitive dentures- same OVD as transitional
9
Q
Metal based dentures in toothwear (Bruxism)
A
- CoCr backing
- difficult
- need to have a wax trial of teeth before making CoCr
