- high caries rate
- perio disease

- high possibility of tooth loss within arch
- prognosis of abutment poor
- length of span too great
- ridge form and tissue loss
- tilting and rotation of teeth
- degree of restoration
- periapical status
- bone loss??

Bridge terminology
A
- abutment: tooth serves as attachment for a bridge
- pontic: artificial tooth which is suspended from abutment teeth
- retainers: extracoronal/ intracoronal restorations that are connected to pontic and cemented to prepared abutment teeth
- connectors: components which connect pontic to retainer
- edentulous span: space between natural teeth that is to be filled by a bridge/ partial denture
- saddle: area of edntulous ridge over which pontic lies
- pier: an abutment tooth which stands between and is supporting two pontics, each pontic attached to a further abutment tooth
- Unit: either retainer/ pontic

Types of bridge design
A
Fixed- fixed bridge
- has a retainer at each end with pontic in the middle joined by rigid connectors
- can be adhesive/ resin retained
- can be conventional
Cantilever bridge
- has a retainer on one side of pontic only
- can be resin retained/ conventional

A
- resin retained bridgework
- adhesive bridge
- minimal prep bridge
- maryland bridge

Advantages of RBB
A
- minimal/ no prep
- no anaesthetic needed
- less costly
- less surgery time
- if fails, less destructive than others

Disadvantages of RBB
A
- metal-shine through
- chipping porcelain
- can debond
- occlusal inteference
- no trial period for pt

Indications for RBB
A
- young patient due to it being less destructive
- good enamel quality
- large abutment tooth surface
- minimal occlusal load
- good for single tooth replacement

Contraindications of RBB
A
- insufficient/ poor quality enamel
- long spans
- excess soft/ hard tissue loss
- heavy occlusal load, ie: bruxism
- poorly aligned, tilted, space teeth
- pt plays contact sports

Direct RBB
A
- very useful for emergency situation
- tooth extracted immediately
- if tooth has been lost traumatically
- ideally using pt’s own tooth
- alternatives: acrylic denture tooth, polycarbonate crown, cellulose matrix filled with composite
