ACUTE TMD:
1. Onset:
2. Locking:
3. Is Jaw Open and cannot close?
4. length? frequency?
5. Malocclusion?
6. Acute sudden malocclusion- diagnoses?
- Onset: Pain is present < 3 months
- Locking: Is jaw Closed & cannot open?
- Is Jaw Open and cannot close?
- How many seconds, minutes, hours, or days for locking? Frequency?
- Malocclusion: was it gradual or sudden? Date first noticed & activity caused it.
- Acute sudden malocclusion- diagnoses myospasm, myositis, fracture, tumor
CHRONIC:
1. Onset:
◦ Ask pt if they have had? if yes?
◦ 2. Jaw locking?
◦ 3. Gradual Malocclusion? likely?
CHRONIC:
1. Onset: Pain is present > 3 months
◦ Ask pt if they have had this pain before.
If YES ask when it began
◦ 2. Jaw locking > 3 months
◦ 3. Gradual Malocclusion > 3 months:
likely arthritis or osteochondma
stabilization splints and nightgaurds as tx’sin osteolysis
◦ Stabilization Splint worn for 6 months (to determine if condylar lysis has stabilized by checking occlusal contacts on splint prior to restorative treatment.
◦ Check monthly to see if occlusion is stable on nightguard.
often used with osteolysis cases to determine stability/progression
RA signs:
* symmetry?
* joints
* morning stiffness ?
* hands?
* systemic?
- symmetric joints involved
- joint inflammation
- morning stiffness longer than 1hr
- spindle swelling of PIP and volar subluxation MCP
- systemic manifestations (fatigue)
osteoarthrhitis signs
- one or two joints invovled
- no inflammation
- moring stiffness <15min
- herbdens nodes of DIP
- bouchards nodes PIP
- no systemic manifestations
myalgia criteria
- one or more of the following: mm pain, mm pain on opening, mm pain on excursion
- mm pain on palpation that DOES NOT REFER
myofascial pain criteria
- regional dull aching pain aggrevated by mandibular function
- trigger points are present that increase the pain AND REFER the pain
arthralgia criteria
- one or more of the following: joint pain report, joint pain on opening or excursion
- joint pain on palpation
- absence of crepitus
- absemce of degen changes on imaging
osteoarthitis criteria
- one or more of the following: joint pain report, joint pain on opening or excursion
- pain on palpation
- crepitus during any movement may be present
- degen changes on imaging
osteoarthrosis criteria
- neither joint pain reported or with palpation of joint
- crepitus during any movement
anterior disc displacement with reduction criteria
- click on open/close
- corrected deviation of mandible to affected side with open/close
- MRI reveals ant displaced disc with reduction upon opening
anterior disc displacement without reduction criteria
- limited opening (assisted) <35 mm
- deflection of mandible to affected side
- MRI would reveal ant displaced disc which does not reduce with opening
