group of disorders characterized by pain in the pre auricular area, TMJ or muscles of mastication; limitations/deviation in mandibular ROM; TMJ sounds during jaw function
temporomandibular disorders
slicking, deviation, fatigue, but no pain and no disability, severe pain with severe disability
signs and symptoms of TMD
causes of TMJ pain
repetitive stress (restriction and compensation) behaviors (posturing)
inflammation of the ligaments/capsule internal derangement of ligament or disc joint arthritis muscles sensitization
potential pain generators
dense fibrocartilage divides the joint into a superior cavity where translation occurs and an inferior cavity where rotation occurs, filled with synovial fluid to lubricate the joint
TMJ disc
limits posterior displacement during jaw opening
temporomandibular ligament
resist excessive medial/lateral displacement of disc
collateral ligaments
suspending mandible and preventing excessive protrusion
accessory ligaments
loud click on opening= disc reduction;
smaller click on closing= disc dislocation
reciprocal clicks
presents with reciprocal clicks
anterior disc displacement with reduction
history of reciprocal clicks that have now stopped
often have limited opening range and referred to as closed lock
anterior disc displacement without reduction
unable to close mouth and referred to as open lock
rare but can occur after prolonged dental procedures when lateral pterygoid becomes overstretched
posterior disc displacement
muscles of mastication
masseter temporalis medial pterygoid lateral pterygoid suprahyoid infrahyoids
jaw pain limited opening popping, clicking locking headaches tension, cervicogenic and migranes dizziness ringing in the ears/ear pain or fullness int he ears swallowing difficulty
TMD symptoms
mobilize all patients its just a restriction of the upper c spine you just need to stretch PT can;t help they are crazy patients patients need braces or surgery
