why does joint click and how are clicks classified
lack of co-ordinated movement between condyle and articular disc
condyle has to overcome mechanical obstruction before full joint movement can be achieved
clicks classified as early middle late
Most common cause of TMJ clicking
Anterior disc displacement with reduction: disc displaced anteriorly by the condyle until reduction occurs
Signs and symptoms of anterior disc displacement
jaw tightness/locking
movement impaired for short period of time until disc reduces
mandible deviation to affected side
TMD pain in morning/ day?
morning- bruxism
day- habits
Treatment
reassurance, soft diet, analgesia- NSAIDs, local heat, eliminate habits, jaw joint and muscle exercises, BRA
Pathogenesis
inflammation of MOM or TMJ secondary to parafunctional habits
trauma- direct/indirect e.g. sustained opening during dental tx
stress- SH important
psychogenic
