TMD Flashcards
collateral ligaments of TMJ
medial goes from disc to medial pole
lateral goes from disc to lateral pole
stylomandibular ligament
- primary function to limit excessive jar protrusion
sphenomandibular ligament
- no significant affect on limiting normal mandibular motion
articular disc
- dense fibrous tissue
- divides joint into superior and inferior compartments
- posterior attachment to the temporal bone through a section of loose retrodiscal tissue
- anterior attachment to the capsule and superior lateral pterygoid muscle
mandibular opening muscles
- inferior lateral pterygoid
- digastric
- secondary muscles: stylohyoid, geniohyoid, mylohyoid
40-60 mm
mandibular closing muscles
temporalis
masseter
medial pterygoid
lateral mandibular movement
- temporalis (ipsilateral)
- medial and lateral pterygoid (contralateral)
5-10 mm
protrusion
5-10 mm
masseter
medial pterygoid
lateral pterygoid
retrusion
temporalis
digastric
hyoid muscles
small mobility
innervation
- trigeminal nerve (cn V)
- opthalmic division (orbit and frontal)
- maxillary division (anterior portion of temple, maxillary, nasal cavity, upper teeth)
- mandibular division (post temple, masseter, mandibular, lower teetch)
mouth opening
- rotation of mandible (condyle) on articular disc (20-25 mm)
- translation of condyle and disc on mandibular fossa (condyle moves anteriorly and inferiorly; anterior surface of mandible moes posteriorly and inferiorly)
mouth closing
- disc and condyle translate together posteriorly
- condyle rotates posteriorly
lateral mouth movement
- unilateral rotation of ipsilateral condyle
- unilateral translation of contralateral condyle
craniomandibular disorder symptoms
- face or jaw pain
- joint sounds
- limitation in jaw movement
- muscle tenderness
- TMJ joint tenderness
- headache
- ear symptoms
- cervical spine disorder
group 1: muscle disorders
- myofascial pain
- myofascial pain with limitations in aperture
group 2: disc displacement
- disc displacement with reduction
- disc displacement without reduction but no limitations in aperture
- disc displacement without reduction and with limitations in aperture
group 3: arthralgia, arthritis, arthrosis
- arthralgia
- OA of TMJ
- osteroarthrosis of TMJ
disc displacement with reduction
S path
disc displacement without reduction
jaw with deviate towards involved side
condylar subluxation
- shallow articular eminence
- lax capsule and ligaments
give clunk sound
jaw will deviate away from side
hypermobility intervention
- stabilization
- teach to not open wide
- teach to limit mouth opening that can be performed in the tongue up position
- teach lateral pole palpation
- isometric exercies
- address muscle weakness
- mandibular muscle relaxation
hypomobility intervention
mobilize
- manual mobilization
- self-mobilization
- intraoral appliance
- stretching
