What is a lagging indicator? (Include 2 examples)
A lagging indicator is something that has already happend that we cannot change. Examples include Lost Time Accidents and Restricted work days, Hours Missed
Why do we measure lagging indicators?
We measure lagging indicators because they are simple - Connected to Outcome - OBJECTIVE and Reliable. We can use these measurements to gauge performance / improvement.
What are LEADING indicators? Provide 2 examples
LEADING indicators are things that haven’t happened yet, therefore you can MANAGE them (i.e. prevent a potential incident or accident from happening) Examples are Programs and training, procedures - physical conditions - inspections, audits, risk assessments,
Who fills out forms 6, 7 and 8?
Form 6 is completed by the worker
Form 7 is completed by the employer
Form 8 is completed by the DR
someone is scheduled to work Monday to Friday and they get hurt on Friday and the dr says two days rest and they’re back Monday is that a lost time accident?
No - because they didn’t miss any work time
Is the FAF - The Functional Ability Form Confidential?
No, ERs can ask for it because they need to be able to see what a limitations a person may have post-accident
If there is an accident WITH LOST TIME / earns less than regular pay, or gets health care treatment (not first aid), how many days do you have to report it?
You have 3 days to report if there IS LOST TIME
If there is an accident with NO LOST TIME, but does recieve modified work at full pay - how many days do you have until you have to report it?
You have 7 days to report if there is NOT ANY LOST TIME

Immediate Causes are the CIRCUMSTANCES that immediately precede the incident

What are some Examples of SUBSTANDARD ACTIONS?
failure to use proper PPE, improper position for
tasks, operating without authority, using defective equipment, etc.
What are some Examples of SUBSTANDARD CONDITIONS?
poor housekeeping, inadequate
guards/barriers, temperature extremes, etc.
What are BASIC CAUSES?
Basic Causes are the REASONS WHY unsafe acts and conditions occurred in the first place
BASIC Causes consist of 2 factors, they are
PERSONAL factors and JOB Factors
