TKR Flashcards
What are the major criteria for infected PJI
1) Two positive results of cultures growing the same organism using standard culture method
2) Sinus tract with evidence of communication to the joint or visualization of the prosthesis
What are the minor criteria for infected PJI in acute
lass <3 weeks
crp >100 mg/L -2
wbc>10000 / lucocyte esteras ++/alpha defensin >1 -3
synovial PMN >90 -2
singel positev cultuer -2
positev histology at surgery ( >5 nutrophis in 5 feelds in 400 magnification ) -3
positev intraoperativ puralance -3 ( canto be used if metal debris present )
What are the minor criteria for infected PJI in chronic
more>3 weeks with symptoms
crp >10 mg/L or D-dimer >860 -2
ESR > 30 -2
wbc>3000 / lucocyte esteras ++/alpha defensin >1 -3
synovial PMN >70 -2
singel positev cultuer -2
positev histology ( >5 nutrophis in 5 feelds in 400 magnification ) -3
positev intraoperativ puralance -3 ( canto be used if metal debris present )
6> INFECTED ,3> NOT INFECTED
What is the order of release in varus Knee
1) Deep MCL Release
2)Release Posteromedial Corner
3)Consider PCL Release/Substitution If Imbalance Persists At This Point
4) Release Semimembranosis (Especially If There Is An Associated Flexion Contracture)
5) Complete Superficial MCL Release / Pes Anserinus ( posterior oblique portion is tight in extension (release if tight in extension)
anterior portion is tight in flexion (release if tight in flexion)
What is the order of release in Valgus Knee
1)posterolateral capsule
2) iliotibial band if tight in extension
3 ) popliteus if tight in flexion (release if tight in flexion)
4) LCL
Wiche classification used in OA of knee
Kellgren
1) Possible osteophytic lipping + doubtful JSN
2) Definite osteophytes + possible JSN
3) Moderate osteophytes + definite JSN + some sclerosis + possible bone end deformity
4) Large osteophytes + marked JSN + severe sclerosis + definite bone end deformity
What increases the Q angle
internal roration or medialazation of femural or tibial componnent
placing the patella lateral
What is the order of release in flexion contracture deformity
1) posterior femoral & posterior tibial osteophytes
2) posterior capsule
3) additional resection of distal femur
4) gastrocnemius muscles (medial and lateral)
