efficacy vs effectivenetss
efficacy is a measure of success of a treatment when done in a clinical seetting/ideal setting
effectiveness is a measure of success of a treatment in everyday practice where subjecst are not excluded based on individual differences, where there is less monitoring by healthcare workes and where patients are generally less adherent than in the hospital seting.

the fact that we don’t usually publish negative outcomes! we are more likely to publish positive outcomes so we are hiding the failed outcoems from our initialy search
