- thyroid cancer - carcinoma vs multiple

what investigations would you order for a thyroid nodule?
- TSH
- ultrasound
what are the high risk features of a nodule on ultrasound?
- hypoechoic
- hypervascular-
- microcalcifications
If suspect a nodule and TSH is normal then
- Biopsy!
If suspect a nodule and TSH is suppressed then
- RAI to check if hot or cold
If nodule is hot then
treat hyperthyroidism

- papillary thyroid carcinoma - PTC
- follicular adenoma
aetiology of thyroid cancer
- external radiation
- family Hx
- high iodine intake,
- obesity, insulin resistance
thyroid carcinomas can be classified as
- well-diff (PTC, FTC) - >90%
- Poor-diff
- anaplastic
features of anaplastic thyroid carcinoma
- p53 mutation
- mean age 65
MTC carcinoma - derived from
Parafollicular cells (secrete calcitonin and CEA)
mutations in MTC cancer
- 80% sporadic
- 20%

which diagnosis can you make on a FNA
- pTC
Most aggressive carcinoma is
medullary! - CEA and calcitonin good markers
What size does the nodule have to be in order to be biopsied?
