what is the nursing process
a systematic method that directs the nurse and client as they determine the need for care, plan and implement the care and evaluate the results; it aims to make the client independent again as quick as possible

- objectives are set for each diagnosis

- physical appearance and hygiene
- body structure
- body movement
- emotional/mental status
assessment for life threatening concerns
a- airway patent
b- breathing normally
c- pulse? BP? Urine output
d- disability: level of consciousness/ behaviour
e- exposure (temp, obvious injury or infection)
pain assessment
p- provoking/palliative: what makes it better or worse
q- quality: what does it feel like
r- region and radiation: where is it? spreading?
s- severity: rate out of 10
T- time of onset/duration: when did it start, how long has it lasted?
u- patients understanding of pain: what do they think it might be, what have they tried?
